Q.    How do I make an appointment with a GOES physician?

A.    To schedule an appointment at any of our locations, please contact our office between 8:00am     and 5:00pm, Monday through Friday at 937-324-3937 or toll free at 1-800-638-2034.

A.    GOES offers emergency ophthalmic care for a wide variety of eye emergencies.   Our ophthalmologists are available 24 hours per day, 7 days per week to examine, treat, and triage     patients.  If you are experiencing an eye emergency, please contact Springfield office at 937-324-3937.

Q.    Where can I learn more about insurance and financial matters related to my care at GOES?

A.    GOES billing office is available from 8:00am to 5:00pm, Monday through Friday, to answer your questions regarding your insurance, billing, and payment information.  Some key that will be helpful are:

  • Bring all insurance information, including insurance cards, when you come to your GOES location.
  • All GOES physicians are Medicare participating providers.
  • A majority of medical insurance plans require pre-authorization for medical services.  Please follow up with you insurance plan or primary care doctor to obtain pre-authorization services prior to the appointment.
  • GOES accepts cash, checks, Visa, Discover and Master Card for payment of services. Co-payments and payment for non-covered services are required at time of visit.
  • Please contact our insurance department at (937) 324-3937 x307 for any questions about insurance.

A.    Yes.  Dr. Fish and Dr. Storck perform laser vision correction surgery.   Please contact our office to schedule your free laser vision correction evaluation.
